Schedule: Mon - Fri, 8am - 5pm (Will be required to work at other sites. Will be required to work / participate on on-call panel and work evenings and/or weekends)

Summary:

Under the direction of the Medical Director, the Nurse Practitioner provides comprehensive medical care to the participant. The NP attends to the acute needs of the participant, while managing the chronic medical problems, and paying attention to the preventive and rehabilitative care required by the participant. The NP incorporates the entire interdisciplinary team as appropriate to provide the medical care in a holistic manner, as is the hallmark of the delivery of care to the participant.

Job Requirements
Education Required (Minimum level of education):
Minimum: Nurse Practitioner License, or Licensed Independent Practitioner
Preferred: Nationally Certified Family Nurse Practitioner.
Certifications/Licenses Required:
Minimum: RN license and CPR Certification
Preferred: MSN Preferred
Experience Required (Minimum level of experience):
Minimum: Demonstrated experience in community clinic and/or Managed Care System.
Experience working with the elderly.
Supervisory Experience
Supervision of nurses.

Equipment Used: Diagnostic and therapeutic spirometry (e.g. EKG)
Verbal and Written Skills Required to Perform the Job: Bilingual (English/Spanish) Preferred. Good oral and written communication
Technical Knowledge and Skills Required to Perform the Job:
Wound Care, ACLS
